Ok first thing... But INSURANCE ALL the time.  Everytime you die, head to get another insurance...  If you die without insurance you loose everything in your sack... So unless you have you antiballast on you, IE as your boots (yes you never buy/build boots), you'll loose it.

So to get it back you have to pay, yes another 1 mil :)

1 - Answer a riddle in a mission wrong (Yes is CAN Happen!)   

this may happen even if you have insurance

2 - Accidently drop it
3 - Die with no insurance
4 - Have it fall out of yer sack cos its too full
